How to Create a Site Form
đź’ˇSite forms let you capture structured information on site: inspection details, damage descriptions, materials used, and more, once linked to an associated form or Word template, turn those answers straight into a finished report. This is set up once in Admin and can then be used across the relevant jobs.
Steps:
Open Site Form Templates
- Go to Admin, then under Jobs select Site Form Templates.

- Here you can view and edit all the site forms that have been created in the system.

Start a new site form
- Select NEW in the upper right corner.

- Enter a Name for the site form.

- If required, choose specific Inclusive Clients, Excluded Clients, or Workflows to control which jobs the site form is used on.

- Add a Description with any additional information.

Build out sections and questions
- Select ADD SECTION and enter a section name.

- Within the section, select ADD QUESTION, then enter the question or heading.

- Use the Type drop-down to set the kind of answer expected — options include Text, Yes No, Option, Rating, Thumb Up Down, Date, Multiple Options, Signature, and Photo.

- Enter a Reference Tag for the question. This doesn't need to be long or complex, but make sure it's clear how each reference tag relates to its question. Reference tags can't contain spaces — use a dash instead (for example,
inspection-date).

- Repeat this for the rest of your sections and questions.

Preview and Save
- Select Preview at any time to check the site form is set up the way you want.
- Once you're happy with it, select Save.

Link an associated template
- Once the site form has been saved, select the Associated Form Template drop-down and choose the form or Word template you want linked to it.
- When someone completes the site form, their answers are generated into that associated form or Word template, using the reference tags you added, making it easy to produce a finished report.

FAQ
Do I need to fill in a reference tag for every question? Yes, if you want that question's answer to appear on an associated form or Word template. Keep tags short, make sure they clearly relate to their question, and use a dash instead of a space.
Can I restrict a site form to specific clients or workflows? Yes, use the Inclusive Clients, Excluded Clients, and Workflows fields when setting up the site form.
What does "Only One Per Job" do? It limits the site form to a single entry per job, rather than allowing it to be filled out multiple times.
Can I check how a site form will look before it's used? Yes, select Preview at any point while building the site form.
How do the answers end up on a report? Once an associated form or Word template is linked, the answers a user enters against each reference tag are generated into the matching fields on that template.
